Appointments
-We require an EKG before
the program is started. The EKG MUST
be done in our office.
-We require that Lab Blood Tests is done IN
OUR OFFICE within the first month of starting
the program. Lab Blood Tests consist
of 5 tests: CMP, CBC, TSH, Fasting Lipid Panel,
and UA.
- Medical evaluations by the Doctor are by
appointments only.
-You may schedule an appointment to meet with
the Doctor anytime while you are on a Plan.
There
will be a physician fee each time you see
the doctor.
-To Cancel a Doctor's appointment, we request
you do so 24 hours in advance. We will be
happy to reschedule you.
Prescriptions:
_ Lost prescriptions will not be replaced,
but each situation may be discussed with our
doctor.
_ Prescriptions are written for four weeks
(28 days), at the end of which you may change
your plan
but will require doctor’s visit. Doctor’s
visit is mandatory every 3 month. Prior to
your prescription
being refilled, you must be re-evaluated by
our staff member.
_ Plans may be combined, but you will need
the doctors approval. Remember that new prescriptions
can only be written every four weeks.
_ Dieting and medication must be stopped if
you are pregnant or trying to become pregnant.
_Call the Doctor/Doctor’s office if
you have any question about your Medications.
_The Florida Law Allows you to consult your
Practitioner about availability of a less
expensive generic equivalent and requirement
of Florida law concerning you Medications.
Fees:
-Our weight loss plan fees are based on four
week intervals. All other parts of your weight
loss program must be used during that four
week time period.
-All fees are due at the time of service and
must be paid in full for the four week time
period.
-We happily accept Cash, Check, Debit, American
Express, Visa or MasterCard payments.
